Lichen planus from childhood to adolescence
A 10-year-old boy was referred to the Pediatric Dentistry/Oral Medicine clinic regarding oral LP. He had a 6-month history of intermittently sore patches on his lips and buccal mucosae, sometimes triggered by spicy foods, along with itchy skin patches on his trunk and limbs. His medical history was unremarkable. Extra-oral examination showed hypopigmentation adjacent to the left eye, Wickham’s striae, hyperpigmentation of the lower lip vermilion border, and dystrophic fingernails. Intraorally, there were lichenoid striations and atrophic patches on the buccal mucosae. He was referred to dermatology, who diagnosed cutaneous LP and vitiligo.
Initial management included clobetasol propionate 0.05% ointment for recurrent lip ulcers and benzydamine hydrochloride 0.15% spray for symptomatic oral relief. Skin and nail lesions were managed with mometasone furoate 0.1% and tacrolimus 0.1% ointments. Over several years, his oral lesions flared intermittently, and he developed widespread cutaneous LP and vitiligo, resulting in post-inflammatory hyperpigmentation. His condition is now stable, and he has been advised by dermatology regarding camouflage techniques for his skin.
Conclusion
This case illustrates the progression and long-term management of pediatric LP into adolescence, accompanied by the co-presentation of vitiligo. Also autoimmune in aetiology, vitiligo affects about 2% of children. Its co-localization with LP is rare and reinforces the importance of careful and effective multidisciplinary input, as both conditions can have long-term symptomatic and cosmetic effects.
