Digital vs. paper consent in UK head and neck surgery: Advancing digital dentistry
Objectives:
This project aims to evaluate the transition from paper-based to electronic consent (e-consent) in head and neck surgery at University College London Hospitals, assessing compliance with key documentation elements and potential impacts on patient safety and medico-legal practices.
Methods:
A retrospective review of over 400 consent forms was conducted across the oral surgery, rhinology, otology, and laryngology departments. Each form was analysed against 15 documentation criteria, including legibility, clarity of procedure description, and presence of patient/clinician identifiers. Statistical significance was determined using a chi-square test.
Results:
E-consent showed superior compliance in several areas, including legibility (100% vs. 46–86%) and recording of patient gender (100% vs. 0–44%). However, the clinician role was absent in all otology, rhinology, and laryngology e-consent forms (0%) compared to 90–100% in paper forms (p<0.05). Additionally, six paper consent forms were not uploaded to patient records, highlighting medico-legal risks.
Conclusion:
E-consent enhances documentation accuracy and compliance but requires refinement in clinician identification. A department-wide training initiative will address these gaps, with future audits evaluating its effectiveness. Before widespread adoption, factors such as accessibility, patient experience, and translation services must be considered.
