Full-arch rehabilitation with tilted implants: A predictable alternative to bone grafting
Background: Edentulism compromises function, esthetics, and quality of life. Traditional implant rehabilitation often requires bone grafting in posterior regions, increasing cost, morbidity, and treatment time. Tilted implant placement has been introduced as a minimally invasive alternative to overcome anatomical limitations.
Objective: To assess the clinical and prosthetic outcomes of full-arch rehabilitation using tilted implants.
Methods: Patients with edentulous arches or failing dentition were treated with four to six implants, including posterior tilted implants to maximize bone use and avoid sinus or nerve involvement. Immediate loading was performed with screw-retained provisional prostheses, followed by definitive prostheses after 6 months. Clinical parameters evaluated included implant survival, marginal bone levels, prosthetic complications, and patient satisfaction.
Results: At 12 months, implant survival was 98%, with mean marginal bone loss below 1 mm. No major prosthetic complications were observed. Patients reported high satisfaction with function, esthetics, and reduced treatment time.
Conclusion: Full-arch reconstruction with tilted implants is a predictable approach that minimizes surgical invasiveness, eliminates the need for grafting, and provides excellent functional and esthetic outcomes.
