An audit on the management of broken/ lost fixed orthodontic appliances
Breakage or loss of orthodontic components poses potential risks of aspiration or ingestion, requiring prompt recognition, appropriate management, and clear documentation in line with British Orthodontic Society (BOS) guidance. A first audit cycle at St George’s Hospital (April–May 2024) identified inconsistent documentation, with only 48% of cases confirming whether broken components were accounted for and limited evidence of patient safety discussions. Following targeted departmental teaching and the introduction of a locally adapted BOS management flowchart, a second audit cycle (April–May 2025) was conducted to evaluate the effectiveness of these interventions.
Re-audit findings demonstrated a significant improvement in compliance, with 81.4% of cases now documenting in line with standards, reflecting enhanced clinician awareness and practice. Continued gaps were noted around recording the cause of breakage and red flag symptom checks. The audit highlights the positive impact of structured education, visual clinical aids, and governance-driven feedback in embedding patient safety principles into routine orthodontic care that may benefit other practitioners.
