Prosthetic rehabilitation with zygomatic implants and immediate loading in cases of severe maxillary atrophy
The use of zygomatic implants represents an alternative to extensive grafting procedures for the management of severe posterior maxillary atrophy.
Objective: To rehabilitate severe cases with advanced posterior maxillary bone atrophy, where patients with conventional complete dentures experience difficulties in achieving functional rehabilitation, or are unwilling and unable to wear them.
Methods: Ten edentulous patients with severe posterior maxillary atrophy underwent prosthetic rehabilitation with the placement of implants both in the anterior maxilla and in the zygomatic bone. All patients were treated with two zygomatic implants using the extrasinus (hybrid zygoma, quad zygoma) technique and at least 3–4 conventional implants. All cases were immediately loaded on the second or third day with a provisional fixed prosthesis. After four to six months, definitive prosthetic rehabilitation was completed with a fixed prosthesis supported by these implants.
Recommendations: Zygomatic implants should be placed only in cases where conventional implants (transnasal, trans-sinus, or pterygoid) cannot be used, as well as in cases where bone augmentation procedures are not feasible.
