Improving post-operative care instructions for orthognathic surgery patients: A quality improvement project
Background: Clear, standardised post-operative instructions are important following orthognathic surgery to support patient recovery. At Bradford Teaching Hospitals, no standardised written instructions were routinely provided at discharge.
Aim: To assess the quality of post-operative advice documented in orthognathic surgery discharge summaries, identify gaps, and implement standardised, orthognathic-specific instructions.
Methods: A baseline audit of 16 orthognathic procedures between September and December 2024 assessed documentation of diet, analgesia, oral hygiene, bleeding, swelling, altered sensation and Hilotherm use. A standardised discharge template was developed and disseminated to trainees. A second cycle of 15 discharge summaries was subsequently reviewed between June and August 2025.
Results: In Cycle 1, documentation was 100% for diet, 93.75% for analgesia, 81.25% for oral hygiene, 31.25% for bleeding and 12.5% for swelling. Altered sensation and Hilotherm advice were absent. Cycle 2 demonstrated 100% compliance for diet, analgesia, oral hygiene and bleeding, with 66.7% for swelling, 66.7% for altered sensation and 53.3% for Hilotherm advice.
Conclusion: A standardised discharge template substantially improved how comprehensive post-operative advice was and it is recommended that other hospitals have these. Further work should address barriers to template uptake and aim for 100% compliance across all instruction categories.
