Wisdom for wisdom tooth: Minimum invasive procedure for mandibular impacted 3rd molar tooth (MIPMIT)
Management of impacted mandibular third molars with surgical removal is very common procedure in day-to-day practice, in which Wards or Modified Wards incisions are commonly performed. In some cases, envelope flap and other such types of flaps are commonly performed. Incision, reflection of flap, osteotomy, suturing is normal protocol which gives more tissue handling, more trauma to tissue & discomfort to patient. It also causes swelling and pain. This surgical procedure is needed in deeply seated impacted mandibular third molar tooth. But I observed in some cases of impaction, if some part of the tooth is clinically visible in the oral cavity, we can modify the technique and we can remove this third molar with minimum invasive conservative approach. From this observation, I modified the technique for surgical removal of partially erupted third molar and inset to avoid incision and bone cutting. I observed, minimum invasive procedures causing less post-operative complications and it definitely reduces the time to perform the procedure. The purpose of this presentation is to present the cases that led to this observation, and present the technique modification in design and execution that I have used.
The individuals demand for conservative approach and stitch-less surgeries is increasing day by day. Through this presentation I suggest conservative technique for management of mandibular third molar impacted tooth. This is a new approach for removal partially impacted molar without surgical intervention & minimum trauma to tissue. This conservative approach gives less instrumentation and reduces time to perform procedure and causes minimum post-operative complications. This MPMIT technique is for the wellbeing of Human Mankind. This technique is copyrighted by Government of India in 2019. Also, this technique got trademark from Government of India by name “MIPMIT Dr. V. H. Nikam’s Technique” in 2021.
